Dear Members of the Jagiellonian University Community,

We would like to kindly inform you that a new agreement has been signed between the Jagiellonian University and PZU Zdrowie for providing private health care services (medical care packages) for Jagiellonian University employees.

The current declarations will expire on 31.01.2025.

Under the agreement, there is the possibility to choose a basic package or an extended package (the ranges of the packages remain unchanged). Both you and members of your families may benefit from the presented offer. Please read the offer presenting options of each package, conditions for participation. All information can be found on the website of the Centre for Personnel Affairs, bookmark ‘Medical care packages’.

In order to join the agreement for providing health care services please fill in proper declarations (attached herein and available on the CSO website) and deliver the declarations to the Centre for Personnel Affairs:

If you wish to join the health care agreement from 01.02.2025, please send back the completed forms to: pakiety.medyczne@uj.edu.pl no later than 20.01.2025. Due to the fact that the dues resulting from joining the medical package will be deducted from monthly salaries, please immediately deliver the signed declarations to the Centre for Personnel Affairs. It is also acceptable to send a declaration signed with an electronic qualified signature to pakiety.medyczne@uj.edu.pl.
